Will my insurance allow me to see Wendy Bongers M.D.?

If your insurance is one of the following insurances, you are most likely covered or in network.
In Network: Medicare, Medicaid, United Health Care, UMR, Aetna, Allina Health, Humana, Medica (Most plans), Blue Cross Blue Shield. Please always call insurance company and ask anyway. There are some insurances within these plan that are exclude certain clinic.
Out of Network: UCare, Health Partners, Prime West, Cigna, Tri West. If you have these insurance, we would love to see you but there may be a higher out of pocket expense for you. Please call us and we can help guide you through this process.
Do I need a referral to see Wendy Bongers M.D.?
The answer is maybe. If we are In Network, it does not mean you can make an appointment and see us. You may be required to get a referral from your primary care doctor to see a dermatologist. We understand the difficulty sometimes navigating insurance but we will help as much as we can. We have found that some plans within Medica and Blue Cross Blue Shield require to referral prior to seeing us. It is easy to do this. Just call your primary care provider and ask them for a referral to dermatology, specifically to One Bread One Body MD in Faribault, MN. Does One Bread One Body M.D. see self pay clients?
Yes, we understand the issues many people are having with insurance and the rising costs of insurance. We have self pay pricing that are reasonable to ensure that you get the care you want. Many people are now making the choice to do self pay instead of having insurance. These prices are clearly marked and you will pay at the end of the visit.
How do I get my medical records sent to One Bread One Body M.D.?
There are two ways to get your medical records sent to One Bread One Body M.D. You can either go to your previous dermatology and ask them to send your records to One Bread One Body MD. This is a form you must fill out in person and you will need our address: One Bread One Body MD, 513 Central Ave N, Suite 1, Faribault, MN 55021. Phone number is 507-301-7602 and fax number is 844-689-2493. The second option is to come into our office and we can have you fill out the form here, please note that we need the name and address of the previous dermatology clinic also.
